Name Correction
Name Proof (ID Proof) & Original Bond.
Address Correction Address Proof only if Premium amount is more than 10,000/- along with Original Bond.

Original Bond

Sum Assured

Original Bond + Photo Copy of Application Form (It will be corrected only if there is data entry Mistake by BALIC Staff.

A.D.B. Change Above 50 Years there will be no A.D.B. benefit.
Gender Only Original Bond required (No Documents Required)
Maturity Date Only Original Bond required (No Documents Required).
D.O.B. Correction Age Proof with Original Bond
Units not allocated Only Original Bond required (No Documents Required).
Nominee Name

a) Original Bond if entire name change along with Proof.
b)Original Bond along with Letter in case of Spelling mistake


Also note that Application Letter of the Customer required along with the Proof.
